摘    要:目的:评价专项整治对医院抗菌药物临床应用的影响。方法:从医院信息系统中导出2011年7月-2012年6月(第1次整治)和2012年7月-2013年6月(第2次整治)全院抗菌药物应用数据,利用Excel对抗菌药物的品种、品规数、消耗量及金额、用药频度(DDDs)、日均费用(DDC)等指标进行汇总和分析。结果:继续深入开展抗菌药物临床应用专项整治活动后,抗菌药物AUD从15.75 DDDs/(100人·天)降到11.93 DDDs/(100人·天),下降了24.25%;抗菌药物费用占药费比例从2.67%降到2.16%,下降了19.10%;门诊患者使用抗菌药物百分率从1.73%降到0.85%,下降了50.87%;住院患者使用抗菌药物百分率从19.71%降到16.99%,下降了13.80%;清洁手术预防使用抗菌药物百分率从9.38%降到9.00%,下降了4.05%;预防使用抗菌药物人均用药天数从1.69 d降到0.83 d,下降了50.89%;介入预防使用抗菌药物百分率从17.74%降到13.22%,下降了25.48%。结论:连续有效的行政与药学干预措施可提高临床合理应用抗菌药物的水平。

Abstract:OBJECTIVE: To evaluate the effects of special rectification on clinical application of antibiotics. METHODS: The application of antibiotics from Jul. 2011 to Jun. 2012 (first rectification) and Jul. 2012 to Jun. 2013 (second rectification) exported from the hospital information system was analyzed by using Excel in respects of the types of antibiotics, specification, consump- tion amount and sum, DDDs, DDC, etc. RESULTS: AUD decreased from 15.75 DDDs/(100 persons-d) to 11.93 DDDs/(100 per- sons.d), decreasing by 24.25%; the proportion of antibacterial drug costs in drug cost decreased from 2.67% to 2.16%, decreas- ing by 19.10% ; the utilization ratio of outpatient antibiotics decreased from 1.73% to 0.85%, decreasing by 50.87% ; the utiliza- tion ratio of inpatient antibiotics decreased from 19.71% to 16.99%, decreasing by 13.80%; prophylactic utilization ratio of antibi- otics in the clean surgery decreased from 9.38% to 9.00%, decreasing by 4.05% ; the average medication days of prophylactic use of antibiotics decreased from 1.69 d to 0.83 d, decreasing by 50.89% ; the proportion of antibiotics in the interventional operation decreased from 17.74 % to 13.22 %, decreasing by 25.48 %. CONCLUSIONS : Continuous and effective administrative and pharmaceutical interventions can promote rational use of antibiotics in the clinic.
