我院乳腺病损切除术围术期抗菌药物使用调查分析

目的:了解我院乳腺病损切除术预防性应用抗菌药物情况,为医院规范抗菌药物的合理使用及建立有效的干预措施提供数据支持。方法:制订抗菌药物的合理使用评价标准,对我院2012年2-9月间的285份乳腺病损切除术患者的住院病历进行合理性评价,其中抗菌药物使用不合理的病历有173份,并对其进行统计分析及评价。结果:173例患者均预防性使用了抗菌药物,抗菌药物的使用率为100%。不合理用药主要表现为:用药时机不恰当171例(98.84%);用药疗程过长149例(86.13%),且最长的达9 d;药物选择起点高1例(0.58%);不合理联用13例(7.51%);超剂量用药56例(32.37%)。结论:我院乳腺病损切除术围术期抗菌药物使用较为规范,但仍普遍存在一些问题,应引起医院高度重视,加大管理力度。

Investigation and Analysis of the Application of Antibiotics in Breast Lesion Resection in Our Hospital during Perioperative Period

OBJECTIVE： To investigate the prophylactic application of antibiotics for breast lesions resection in our hospital, and to provide data support for rational use of antibiotics and the establishment of effective intervention measures. METHODS： Evaluation criteria for rational use of antibiotics were established; 720 inpatient medical records of breast lesion resection in our hospital from Feb. to Sept. in 2011 were analyzed; 173 cases of irrational antibiotics use were analyzed statistically and evaluated. RESULTS： There were 173 cases of prophylactic application of antibiotics with utilization rate of 100%. The main unreasonable manifestation was that medication timing was inappropriate in 171 cases （98.84%） ; medication time was too long in 149 cases （86.13%）, the longest was 9 days; the initiation level for medication was higher than normal in 1 case （0.58%） ; there were 13 cases of unreasonable drug combination （7.51%） and 56 cases of over dose （32.37%）. CONCLUSIONS： The prophylactic use of antibiotics during perioperative period of breast lesion resection is standard basically in our hospital, but there still are some problems. Great attention should be paid to rational use and management of antibiotics.
