某院儿科万古霉素临床应用调查

目的：了解万古霉素在某院儿科中的使用情况。方法：调查2012年某院100例使用万古霉素病例，以《抗茵药物临床应用指导原则》、万古霉素临床应用中国专家共识（2011版）及《万古霉素在儿童MRSA的治疗地位》为依据，就其用药指征、用法用量及病原学检查等方面进行调查分析。结果：100例病例中，患儿平均年龄为（23．1±16．9）月，平均住院时间（26．4±16．6）d，应用万古霉素平均天数（9．6±5．2）d，按病原学检查结果用药33例，有效率71％。但该院应用万古霉素存在诸多问题，如经验性用药、适应证过宽、联合随意性大及用药时间过长或过短等。结论：针对临床应用万古霉素存在的问题，应建立特殊使用级抗菌药的限制性措施，严格控制用药指征，减少经验用药，以提高临床疗效。

Investigation of the Clinical Application of Vancomycin in Pediatrics of One Hospital

Objective ： To invstigate and evaluate the clinical use status of vancomycin for hospitalized patients in one hospital. Methods ： According to Guiding Principles of Clinical Application of Antibacterials , Vancomycin Clinical Appli-cation of Chinese Expert Consensus （2011 edition） and Vancomycin in the Treatment of MRSA in Children, a total of 100 case histories of patients in 2012 were invstigated in order to learn the facts about the madication indications, usage and dosage, etiological examinations and so on. Results：The average age of patients was （23.1 ± 16.9） m; the average length of stay in hospital was （26.4 ± 16.6） d; the average use days were （9.6 ± 5.2 ） d. The pathogen culture and susceptibili-ty test were performed in 33 cases and the total effiency rate was 71%. There were still some problems in irrational use of vancomycin such as empiric vancomycin therapy, excessively wide indications, optional drug combination and too long or too short treatment duration. Conclusion： In order to solve the exposed problems in vancomycin application, some restrici-tive measures should be established about the use of special antibacterials. The management of the use of antibiotics shoud be strengthened to promote rational clinical use of drugs.
