隐源性机化性肺炎7例分析

目的提高对隐源性机化性肺炎(COP)的诊治水平。方法对7例诊断明确的病例进行总结,探讨COP的临床、病理、影像学特点及治疗和预后。结果 7例中女性5例,男性2例,肺功能6例显示限制性通气功能障碍,1例患者为混合型通气功能障碍。7例患者均被误诊,影像学示"五多一少"特点,使用抗生素治疗无效,肺活检而明确诊断,糖皮质激素治疗有效,随访示预后好。结论临床表现及影像学特征和诊疗经过,对诊断有一定的提示作用,确诊依靠肺活检病理检查。

Analysis of 7 cases of cryptogenic organized pneumonia

Objective To improve the ability of diagnosis and treatment of cryptogenic organized pneumonia. Methods 7 diag- nosed patients were selected to analyze their clinical characteristics, pathology, imaging features, treatment and prognosis. Results There were 5 male patients and 2 female patients. Through lung function test, 6 cases showed a restrictive ventilatory defect, and 1 case showed impairment of carbon monoxide transfer factor. All of the 7 cases were misdiagnosed, and the antibiotics were ineffective. Their pathologies were confirmed by lung biopsy and the conditions of the 7 patients turned better treated with glucocorticoid. Conclusion The clinical manifestations and imaging features can be taken as the suggestions of the diagnosis of patients with cryptogenic organized pneumo- nia, but the confirmation should be based on lung biopsy.
