大黄与谷氨酰胺对肠缺血再灌注大鼠肠道保护作用比较研究

摘    要:目的 观察肠缺血再灌注损伤时,大黄和谷氨酰胺对大鼠肠黏膜结构和功能的影响,比较两者的肠道保护作用.方法 40只雄性SD大鼠随机分为大黄组、谷氨酰胺组、对照组和伪手术组,前3组建立肠缺血再灌注损伤模型,予持续全胃肠外营养(TPN),每日分别灌胃大黄液、谷氨酰胺和生理盐水.伪手术组分离肠系膜上动脉但不夹闭,自由饮食.收集尿液、小肠、肠系膜淋巴结和门静脉血分别检测小肠通透性、小肠形态学、小肠细菌移位.结果 (1)对照组肠黏膜通透性较伪手术组明显增大(P<0.005),大黄组和谷氨酰胺组则与伪手术组相似.(2)对照组小肠绒毛高度,黏膜厚度显著低于伪手术组(P<0.005),大黄组和谷氨酰胺组优于对照组;小肠全层厚度各组间无显著性差异.(3)对照组培养阳性率高于伪手术(P<0.005),大黄组和谷氨酰胺组阳性率则低于对照组(P<0.01,P<0.05);伪手术组和大黄组门静脉血16s rRNA阳性率低于对照组(P<0.01,P<0.05),谷氨酰胺组则与对照组无统计学差异.结论 肠缺血再灌注损伤使大鼠小肠黏膜结构破坏和通透性增加,细菌移位;肠内使用谷氨酰胺或大黄均有肠道保护作用.对于保护小肠绒毛高度、黏膜厚度及降低肠道通透性,大黄与谷氨酰胺效果相仿.作者单位225001 扬州,江苏省苏北人民医院通讯作者谈定玉,电子信箱webtan1981@gmail.com;Tel13665254359大黄能显著降低小肠细菌移位.

Gut Protective Effect of Rhubarb and Glutamine on Intestinal Ischemia Reperfusion Rats

Abstract:Objective To observe and compare the protective effect of rhubarb and Gln on morphology and permeability of rats intestine after SMA ischemia reperfusion.Methods 40 male rats were divided into rhubarb ,Gln, control and pseudosugery group(n=10)randomly. The intestinal I/R model of rhubarb ,Gln and control group rats was established and TPN was done.Rhubarb ,Gln and normal saline were given orally in rhubarb,Gln and control group respectively. SMA was decoherenced but not blocked in control group. Urine ,intestine,MLN and portal vein blood were collected for L/M,morphology and bacterial translocation study.Results (1)L/M was significant high in control group compared with pseudosugery group(P<0.005), Rhubarb and Gln groups were similar to pseudosugery group.(2)The villi height and mucosa thickness of small intestine in control group were significantly lower than pseudosugery group(P<0.005).The villi height of Rhubarb and Gln groups were higher than that of the control group. There was no statistically difference among 4 groups in full thickness.(3)Positive rate of culture in control group was significantly higher than that of the pseudosugery group(P<0.005), Rhubarb and Gln groups were lower than control group(P<0.01,P<0.05). Positive rate of 16s rRNA gene in pseudosugery and rhubarb groups were lower than that of the control group(P<0.01,P<0.05),Gln group was also lower but there was no statistically difference.Conclusions The intestinal I/R injury can influence mucosa structure of small intestine,increase the intesital permeability and facilitates bacterial translocation. Both rhubarb and Gln can protect the gut.There are similar effect of rhubarb and Gln on morphology and permeability of rats intestine. Rhubarb can prevent bacterial translocation obviously.
