小腿皮神经营养血管皮瓣的术后护理

目的 探讨以皮神经营养血管为成活基础的新型皮神经营养血管皮瓣的术后护理特点。方法 1998年－2000年3月，采用小腿皮神经营养血管皮瓣修复小腿中下段及足跟与足踝部深度创面21例22处，强化和改进术后护理：体位的选择；观察皮瓣方法的改进；血运障碍的处理；预防血管痉挛，保持血管通畅；废用性功能障碍的预防。结果 21例22处皮瓣全部成活良好，伤口一期愈合，术后外形与功能恢复满意。结论 皮神经营养血管皮瓣为深度创面的修复提供了新的手段，术后护理是皮瓣成活的重要环节，临床护理必须抓住上述特点，落实健康指导和护理措施。

Postoperative nursing of patients with skin flap grafting of crural neurocutaneous vasa vasorum

ObjectiveTo probe into the nursing characteristics of skin flap grafting of crural neurocutaneous vasa vasorum (CNVV).Methods22 cases of 21 patients from 1998 to March 2000 received restorations in deep wounds of medium lower segment of leg, heels and ankle joints with CNVV. Postoperative nursing was intensively enhanced in view of selection of postures, improvement of observing the grafted skin flap, handling of barriers of blood flowing, prevention of blood vessel spasm and prevention of disuse dysfunction.ResultsIn the 22 cases, all of the grafted flaps grew well and the wounds healed up. Postoperative contours and recovery of functions were satisfactory.ConclusionThe skin flap grafting with new type of CNVV provides a novel measure to repair deep wounds. Still whether the skin flaps grow or not exists in postoperative nursing. Clinical nursing must aim at the nursing characteristics in order to put health guidance and nursing measures into effect.
