丙泊酚联合米索前列醇用于人工流产术的临床观察

摘    要:目的观察丙泊酚联合米索前列醇实行无痛人工流产术的临床效果。方法 400例实行无痛人工流产的早孕患者随机分为两组。A组(200例):术前1h舌下含服米索前列醇400μg;B组(200例):术前不服任何药物。两组术中以2.0mg/s的速度单次静注丙泊酚2.5mg/kg,必要时追加0.5mg/kg。观察宫口松弛情况、术中出血量、追加用药例数、手术时间、苏醒时间、自行离床时间;观察丙泊酚推药前、推药后2min及手术结束时的血压、心率、呼吸、血氧饱和度的变化。结果两组间的宫口松弛、术中出血量、追加药例数、手术时间、苏醒时间及自行离床时间比较差异有统计学意义(p<0.05)。与麻醉前比较,两组推药后2min其血压、心率、呼吸、血氧饱和度差异均无显著性(p>0.05);手术结束时B组的血压、心率、血氧饱和度下降明显,呼吸增快,差异有统计学意义(p<0.05);手术结束时A组也有血压、心率、血氧饱和度下降、呼吸增快,但差异无统计学意义(p>0.05)。结论丙泊酚联合米索前列醇实行无痛人工流产术可减少丙泊酚术中用量,降低丙泊酚对循环、呼吸的抑制作用。

THE CLINICAL OBSERVATION ON MISOPROSTOL AND PROPOFOL IN INDUCED ABORTION

Abstract:Objective To observe the effect of combined application of misoprostol and propofol in painless induced abortion. Methods Four hundred early pregnancy patients undergoing painless abortion were divided randomly into two groups. Group A:received 400 ug misoprostol before operation;Group B:without misoprostol. Both given propofol 2. 5 mg/kg( the single dosage)to induce anesthesia. Speed of injection was kept 2.0 mg/s. 0. 5 mg/kg propofol was given according to the situation during operation. Following parameters observed: the loosening of cervical, blood loss of intraoperative hemorrhage, the anesthetic effect, the addition cases of drug, the time required for preg- nancy interruption, for analepsia, for consciousness sober, bloed pressure, heart rate, respiratory, saturation oxygen before injection, two minutes after injection, and after operation. Results There is significant difference on loosening of cervical, blood loss of intraoperafive hemorrhage, the anesthetic effect, the addition cases of drug and the time required for pregnancy interruption, for analepsia, for sober between Group A and Group B(p 〈0. 05). There is difference significant on blood pressure,heart rate ,respiratory ,saturation oxygen between preoperation and postopem- tion in Group B (p 〈 0. 05) ,but not in Group A. Condusion The combined application of misoprostol and propofol in painless induced abortion can reduced the dosages of propofol and the suppressive effect of propofol on circulation and respiration.
