脑源性神经营养因子在阻塞性呼吸睡眠暂停综合征患者血清中的表达及临床意义

摘    要:目的探讨脑源性神经营养因子(BDNF)在阻塞性呼吸睡眠暂停综合征(OSAHS)患者血清中的表达状况及临床意义。方法20名健康对照者及42名OSAHS患者纳入研究。根据呼吸暂停低通气指数(AHI)将患者分为轻度OSAHS组(n=12)、中度OSAHS组(n=19)与重度OSAHS组(n=11)。记录患者的AHI、嗜睡量表评分(ESS)、最低氧饱和度(LSaO2)。采用酶联免疫吸附试验(ELISA)法检测所有被研究者血清中的BDNF水平。结果相比健康对照组,OSAHS组血清中BDNF水平明显升高(P〈0.01),OSAHS越重,血清中BDNF越高(P〈0.01);血清中BDNF水平随AHI指数的增加而升高,呈显著正相关(r=0.815);与ESS亦呈正相关(r=0.631),与LSaO2呈负相关(r=-0.723)。结论BDNF在OSAHS患者中显著上升,与AHI、ESS呈正相关,与LSaO2呈负相关;BDNF可以作为OSAHS的重要预测指标。

Abstract:[ Objective ] To investigate the expression status of brain-derived neurotrophic factor ( BDNF ) in serum of patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) and its clinical value. [ Methods ] 20 normal controls and 42 OSAHS patients were enrolled in this study. According to the apnea hypopnea index ( AHI ) , patients were divided into 3 groups : mild OSAHS group ( n = 12 ), moderate OSAHS group ( n = 19 ) and severe OSAHS group ( n = 11 ). AHI, Epworth sleepiness scale ( ESS ) scores and the lowest oxygen saturation ( LSaO2) of patients were recorded. The levels of serum BDNF were measured by ELISA. [ Results ] Compared with the normal control group, the levels of serum BDNF in OSAHS groups increased significantly ( P 〈0. 01), and the serum BDNF level elevated with the severity of OSAHS ( P 〈 0.01 ). Linear correlation analysis suggested that serum BDNF levels were positively correlated with the AHI ( r = 0. 815 ) and ESS ( r = 0.631 ), but negatively correlated with the LSaO2 (r = -0. 723). [ Conclusion] The serum BDNF levels in OSAHS patients increase significantly, which are positively cor- related with AHI and ESS, but negatively correlated with the LSaO2. So BDNF can be used as an important predictor of OSAHS.
