摘 要: | 目的探讨吲哚氰绿(indocyanine green,ICG)排泄试验在急性肝损伤肝功能评估中的价值。方法将32只SD大鼠随机分为4组,腹腔注射生理盐水或不同剂量D-氨基半乳糖(200mg.kg-1、700mg.kg-1或1200mg. kg-1)诱导急性肝损伤,于造模后24h测定吲哚氰绿15分钟滞留率(ICGR15)、肝脏病理学积分及两者的相关性。另将48只SD大鼠随机分为6组,腹腔注射450mg.kg-1D-氨基半乳糖,于0h和造模后6h、12h、24h、48h和72h分别测定ICGR15、肝脏病理学积分及两者的相关性。结果对照组和不同浓度D-氨基半乳糖组病理学积分分别为2.71±0.04、18.40±5.05、79.71±16.98和102.50±18.69分,ICGR15分别为3.8±0.5%、4.0±0.6%、17.3±4.3%和21.5±3.4%,ICGR15与肝脏病理学评分呈显著正相关(r=0.7102,P〈0.001);在D-氨基半乳糖(450mg.kg-1)处理前(0h)和处理后6~72h肝脏病理学积分分别为0.40±0.07、2.86±1.85、5.90±0.95、8.01±0.43、3.66±0.34和2.17±0.40分,其中处理后6h和72h组病理学积分显著高于处理前(P〈0.05),而12h、24h和48h组病理学积分显著高于6h组(P〈0.01)。ICGR15分别为4.8±0.6%、6.1±1.2%、6.9±1.6%、17.0±1.3%、12.5±7.3%和5.6±2.8%,其中12h组、24h组和48h组ICGR15显著高于6h组(P〈0.05);不同时间处理组ICGR15与肝组织病理学积分也呈正相关(r=0.6970,P〈0.001)。结论 ICGR15可以动态和准确地反映急性肝损伤时肝脏的贮备功能。

Abstract: | Objective To evaluate the liver function reserve by indocyanine green (ICG) clearance in rats with acute liver injury. Methods Thirty-two male SD rats were randomly divided into four groups. The rats re-ceived saline or different doses of D-galnactosamine (200mg.kg-1,700mg.kg-1 and 1200mg.kg-1) injected intraperi-toneally to induce acute liver injury. ICG retention rates at 15min (ICGR15)and histopathological scores were ob-tained and the correlation between these two parameters was calculated 24 hrs after injection. Forty-eight male SD rats in the second experiment were randomly divided into six groups and received D-galnactosamine at dose of 450 mg.kg-1 injected intraperitoneally at six hour interval for 72h. ICGR15 and histopathological scores were obtained and the correlation between these two parameters was calculated. Results The histopathological scores of liver tissues in the controls or in the three models were 2.71±0.04,18.40±5.05,79.71±16.98 and 102.50±18.69, respectively and the ICGR15 were 3.8±0.5%,4.0±0.6%,17.3±4.3% and 21.5±3.4%,respectively;The histopathologi-cal scores were positively correlated with ICGR15 (r=0.7102,P〈0.001);The histopathological scores of liver tissues before(0h)or after(6,12,24,48,72h)D-galnactosamine injection were 0.40±0.07,2.86±1.85,5.90±0.95,8.01±0.43, 3.66±0.34 and 2.17±0.40,respectively;The scores at 6h and 72h were significantly higher than that at baseline (0h,P〈0.05),while scores at 12h,24h and 48h were significantly higher than that at 6h;The ICGR15 at different time points were 4.8±0.6%,6.1±1.2%,6.9±1.6%,17.0±1.3%,12.5±7.3% and 5.6±2.8%,respectively,and the ICGR15 at 12h,24h and 48h were significantly higher than that at 6h;The histopathological scores was also positively correlated with ICGR15 (r=0.6970,P〈0.001). Conclusion ICG clearance test is a dynamic and effective indicator of liver functional reserve in rats with acute liver injury. |
