应用螺旋CT重建技术诊断主动脉瘤

摘    要:目的:探讨螺旋CT各种重建技术在主动脉瘤病变诊断中的应用及其价值。方法:对15例主动脉疾病患者行螺旋CT增强扫描,比较3-D(相当于表面覆盖显示即SSD)、4-D、多平面重建(MPR),最大密度投影(MIP)和血管CT仿真内镜(CTVE)的诊断价值。结果:3-D、4-D能显示病变全貌,动脉瘤的大小、范围、夹层内膜片及其与动脉分支的关系,尤其能显示破口位置。MPR能显示附壁血栓、真假腔和内膜片。MIP可清楚显示管壁钙化和动脉瘤范围、形态。CTVE可清晰显示内膜片、真假腔和血管狭窄。结论:综合应用螺旋CT 各种重建技术诊断主动脉瘤,是一种安全可靠的诊断手段,基本上可取代X线血管造影,对临床治疗,尤其急诊手术具有重要指导意义。

Abstract:Objective To explore the diagnostic value of spiral CT angiography for aortic aneurysms. Methods Fifteen patients with aortic aneurysms were scanned by spiral CT. 4 D,3 D (shaded surface display), MPR (multiplanar reconstruction), MIP (maximum intensity projection) and CTVE(CT virtual endoscopy) were used. Results 4 D and 3 D showed the panorama of lesion, size and extent of aneurysm, intimal flap of dissection and the relation with arterial branches,especially the location of laceration. MPR could display murral thrombus, intimal flap, ture and false lumen. MIP could show the wall calcification ,extent and shape of aortic aneurysms clearly. CTVE could reveal the ture and false lumen, intimal flap and stenosis of arterial branches. Conclusion Application of combined techniques of spiral CT can basically replace X ray angiography in diagnosis of aortic aneurysms.It provides another reliable diagnostic method, and has clinical importance for treatment, especially for emergency surgery.
