经迷路后入路岩骨磨除范围的解剖学研究

目的 确定经迷路后入路中岩骨磨除范围的一种方便、实用、安全的方法。方法 观察20侧后半规管的位置，并测量其与乙状窦前缘、内耳门后下缘、岩骨表面的距离和乙状窦前缘、内耳门后下缘和外耳道后上棘的两两间距离，制出这些结构间的位置关系简图，并经过数学计算找到岩骨表面可以磨除的最前点。结果 内耳门后下缘、后半老人家管中点和外耳道后上棘基本处于同一水平面上，后半老人家向与乙状窦前缘、内耳门后下缘、岩骨表面的距离和乙状窦前缘到内耳门后下缘、乙状窦前缘到外耳道后上棘、骨耳孔后下缘到外耳道后上棘的距离分别为8.67mm、15.64mm、3.47mm、22.07mm、12.42mm、25.37mm。结论 内耳门下后缘水平内耳门下后缘后12.5mm或乙状窦前缘前10mm处和外耳道后上棘的连线可用来确定岩骨后部的磨除范围。

Abrading range of the petrous bone in the retrolabyrinth approach

Objective To find a simple, practical and safe method to define the abrading range of the petrous bone in the retrolabyrinth approach. Methch We observed the situation of the post - semicircle canal (PSC), measured the distances between PSC and the sigmoid sinus (SS), PSC and the internal auditory meatus (IAM), PSC and the posterior surface of the petrous be, SS and IAM, SS and Henle's spine, IAM and Hedle's spine. Then we sketched the locative relationship of those structures and found the forefront point on the posterior surface of the petrous bone, which could be abraded. Results The middle point of PSC, interiolaterior lip of IAM, frontal edge of the sigmoid sinus and Henly's spine located on the same level plane, and PSA lying on the center, the distances between PSC and SS, PSC and IAM, PSC and the posterior surface of the petrous, SS and IAM, SS and Henle's spine, IAM and Henle's spine were 8. 67 mm, 15. 64 mm, 3. 47 mm, 22. 07 mm, 12.42 mm and 25. 37mm. Conclusion We can safely define the abrading range by the posterior part of the petrous, which is rearward the line between Henle's spine and the point on the level line through the interiolaterior lip of IAM 12.5 mm behind the interiolaterior lip of IAM or 10 mm before the frontal edge of the sigmoid sinus.
