幽门螺杆菌感染胃粘膜癌变过程中基因表达的研究

摘    要:目的 进一步探讨幽门螺杆菌(Hp)在胃癌发生发展过程中作用的分子 机制。方法 胃镜活检及手术切除组织病理诊断为胃粘膜病变327例,免疫组化法检查抑癌基因与凋亡调节基因蛋白的表达。Hp阳性由CLOtest结合病理染色/∧14C尿素呼吸试验而确定。结果Hp阳性组慢性萎缩性胃炎、肠化或异型增生p16阳性表达率均显著低于Hp阴性组(P<0.05、P<0.01)。而Hp阳性组肠化或异型增生bcl-2阳性表达率均显著高于Hp阴性组(P均<0.05)。p53阳性表达率在Hp阳性及阴性胃癌组之无显著性差别。结论 在胃癌发生的早期即存在较明显的p16基因表达低下bcl-2基因过度表达,并与Hp感染有一定的关系。p53基因过度表达是胃癌发展过程中较晚期事件,下Hp感染无明显相关性。

Abstract:Objective To investigate the molecular mechanism of Helicobacter pylori(Hp) infection in gastric tumorigenesis.Methods 327 cases of gastric mucosal lesions underwent endoscopy and surgical resection.ABC immunohistochemical staining was used to detect the expression of p53,p16,and bcl 2 proteins.Hp was determined by CLOtest combined with pathological staining/14C urea breath test.Results The positive results of p16 expression of chronic atrophic gastritis,intestinal metaplasia or dysplasia with positive Hp were significantly less than those with negative Hp( P <0 05, P <0 01),In contrast to p16,bcl 2 expression of intestinal metaplasia or dysplasia with Hp positive was significantly higher than that with Hp negative.(Pall<0 05).No significant difference for the positive results of p53 expression of gastric carcinoma between Hp positive and Hp negative ( P >0 05).Conclusion p16 or bcl 2 gene expression alteration might play a role in the early development/promotion of gastric carcinoma and is associated with Hp infection.It seems that Hp infection is not related to p53 gene alteration because p53 gene overexpression is relatively later event in stomach tumorigenesis.
