彩色多普勒超声诊断胎儿脐带绕颈的临床价值

目的 探讨彩多普勒超声诊断胎儿脐带绕颈的临床价值。方法 根据彩色多普勒超声对620例胎儿脐带绕颈的观察。分析脐带缠绕方式，缠绕周数并观测脐带动脉的血流速度和胎盘阻力指数S/D值，并与分娩结果对比。结果 随着孕周的增加，彩色多普勒超声诊断胎儿脐带绕颈的敏感性上升，35w-待产前、28-34W、20-27W敏感性分别为97.1%、93.0%、62.8%，脐动脉S/D值能够估价胎盘血管阻力和预测胎儿的预后，结论 彩色多普勒超声在诊断胎儿脐带绕颈中起了重要作用。

Diagnostic value of umbilical cord around the neck of fetal by color Doppler ultrasound

Purpose To investigate the value of Color Doppler ultrasound in diagnosis umbilical cord around the fetal neck. Methods Color doppler ultrasound was used to observe the type and circles of umbilical cord around neck and to measure the resistance index of placental vessels quantitatively (S/D) in 620 cases. All cases had been followed postpartum.Results During different gestational period, the sensitivity in diagnosis umbilical cord around the fetal neck were 62.8% (20 - 27week) ,93.0%(28 - 34week) ,97.1%(parturient)respectively. So it was the most sensitivity at parturient. The umbilical artery S/D value could assess the resistance of placental vessels quantitatively and placental function, thus to predicate the fetal well being. Conclusion Color doppler ultrasound had an important clinical value in the diagnostic umbilical cord around the fetal neck.
