摘    要:目的分析心电监护在低钾型周期性麻痹(HOPP)患者补钾中的意义。方法将我科收治的87例HOPP患者随机分为两组:试验组(50例)行心电监护,对照组(37例)行心肌监护,给予积极补钾治疗,每小时测定血钾和肌力一次。观察两组患者补钾后的血钾浓度及高钾T波例数。结果补钾1 h后,室性心律失常均消失。补钾2 h后,患者血钾水平普遍上升,补钾过程中未出现严重心律失常。心电监护T波正常时血钾水平低于肌力恢复达到4级时血钾水平(P<0.05)。结论心电监护在HOPP患者补钾的监护中高效安全,T波改变的识别是监护技术的关键。

Abstract:Objective To investigate the clinical value for electrocardiogram(ECG)monitoring in potassium supplementing of patients with hypokalemic periodic paralysis(HOPP).Methods 87 patients of HOPP in our office were divided into two groups:experiment group(50 cases)and control group(37 cases).The experiment group accepted ECG while the control group accepted cardiac muscle monitor during potassium-supplement.ECG was needed when supplementing potassium.Hypokalemia and muscle strength were checked one time each hour.The serum potassium and cases of hyperkalemia T wave after potassium supplement were observed.Results After 1 h potassium supplementing,all ventricular arrhythmia disappeared.After 2 h,serum potassium levels generally increased,no severe arrhythmia appeared during the potassium supplementing process.Serum potassium level during normal ECG T wave was lower than that when the muscle strength recovered to grade 4(P<0.05).Conclusion ECG monitoring in potassium supplementing of HOPP patients is efficient and safe,and T-wave changes identification is the key to the monitoring technology.
