新生儿红细胞免疫功能的测定

摘    要:目的 探讨红细胞免疫功能在新生儿期的变化及相关因素。方法 随机选择 2 0名足月正常产的新生儿为新生儿组。采用肝素抗凝脐带血检测红细胞C3b补体受体黏附率 (RC3bR)和红细胞免疫复合物黏附率 (RICR) ,使用普利生LBY—N6B全自动血流变仪 ,用肝素抗凝脐带血检测血液流变学。同时选择 2 0名健康儿童为正常对照组。所获资料用 x±s表示 ,并行组间均数比较 ,新生儿组RC3bR与血液流变学各项指标行直线相关统计学处理。结果 新生儿组RC3bR、RICR与对照组比较 :RC3bRt =0 79,RICRt =1 73,P >0 0 5 ,差异无统计学意义。新生儿组血液流变学与对照组比较 :全血黏度t =2 37,P <0 0 5 ;红细胞聚集指数t =3 0 0 ,红细胞变形指数t=2 85 ,红细胞压积t=8 76 ,血沉t=15 0 1,均P <0 0 1,差异有统计学意义。新生儿组RC3bR与血液流变学各项指标直线相关统计学处理结果 :全血黏度r =- 0 77,血浆黏度r =- 0 75 ,P <0 0 1;红细胞变形指数r=0 5 3,P <0 0 5 ,差异有统计学意义。提示RC3bR与全血黏度、血浆黏度呈负相关 ,与红细胞变形能力呈正相关。结论 新生儿出生时 ,红细胞免疫功能已发育成熟 ,任何原因导致的血黏度增高、红细胞变形能力下降 ,均可使红细胞免疫功能降低。

Abstract:Objective Studying the changes of erythrocytic immune adherent function in normal newborn.Methods Twenty newborns of normal term birth were selected at random,Their heparin-anticoagulant cord blood samples were collected,the adherent rate of complement-Ⅲ receptor on the surface of RBC (RC 3b R) and the immune complex adherent rate of RBC (RICR) were examined with Guo′s method;hemorrheological data were examined with Pulisheng automatic machine,20healthychildrenagedfrom3to14yearswerechosenasthecontrolgroup.Results Their RC 3b R and RICR showed nodifferencebetweenthenewbornsandthecontrolgroup (RC 3b R t=0.79,P>0.05,RICR t=1.73,P>0.05),but the difference in hemorrheologic data showed a great significance between the two groups whole blood viscosity t=2.37,RGI t=3.00,RCI t=2.85,HCT t=8.76,ESR t=15.01,all P<0.01).Inthenewborngroup,therewasapositivecorrelationbetweenRC 3b RandRBCdeformability (r=0.53,P<0.05) ,negativecorrelationbetweenRC 3b Randwholebloodviscosityorplasmaviscosity(wholebloodviscosity r=-0.77,r=-0.75,all P<0.05).Conclusion Erythrocytic immune function has matured in a newborn infant,but an increased blood viscosity and/or a decreased RBC deformability that are induced by any causes can suppress the erythrocytic immune function.
