团体心理治疗对乳腺癌改良根治术后患者焦虑抑郁情绪及生命质量的影响

目的 探讨团体心理治疗对乳腺癌改良根治术后患者情绪状态和生命质量的干预效果.方法 将70例乳腺癌改良根治术后的患者采用随机数字表法分为试验组和对照组.对照组接受常规化疗,试验组除接受化疗外同时参加每周1次,共8次的团体心理治疗.比较2组患者治疗前后抑郁、焦虑水平和生命质量有无差异.结果 1.经过团体心理治疗后,试验组患者抑郁、焦虑和食欲丧失评分分别为(45.08±7.25)分,(42.23±4.72)分,(26.88±20.04)分],低于对照组分别为(50.15±5.82)分,(49.00±7.07)分,(42.39±29.30)分],差异具有显著性(P＜0.01);疲倦评分显著低于对照组分别为(41.75±19.65)分,(48.31±23.62)分](P＜0.05);社会功能、总健康状况和形体领域评分分别为(65.59±23.93)分,(54.30±33.05)分,(64.78±21.91)分],高于对照组分别为(57.47±29.66)分,(48.38±29.30)分,(43.28±21.91)分],差异具有显著性(P＜0.01).2.试验组患者团体治疗前后抑郁、焦虑评分比较,差异具有显著性(P＜0.01).结论 团体心理治疗可以缓解乳腺癌改良根治术后化疗患者的焦虑和抑郁情绪,提高整体生命质量.

The effect of group psychotherapy on mood and quality of life to patients who underwent modified radical mastectomy

Objective To investigate the effectiveness of group psychotherapy to patients with breast cancer who underwent modified radical mastectomy.Methods 70 patients randomized to Group Psychotherapy (GP)or control group for an 8-week observation.Rating Scale was used to compare independently both at baseline and at the end of 8th week,including the SDS,SAS,QLQ-BR53(quality of life questionnaire for breast cancer patients).Results 1.Compared to control,after 8-week GP,the experiment group had less depression and anxiety (SDS:45.0±7.25 vs 50.15±5.82,P＜0.01;SAS:42.23±4.72 vs 49.00±7.07.P＜0.01).showed a significant improvement in social functioning and global health status(65.59±23.93 vs 57.47±29.66,P＜0.01:54.30±33.05 vs 48.38±29.30,P＜0.01)and level of body image(64.78±21.91 vs 43.28±21.91,P＜0.01).2.Compared to baseline,the experiment group had significant reduction in the score at depression and anxiety(P＜0.01),showed a significant improvement in physical sufferings(P＜0.01).Conclusion GP can lessen depression and anxiety,and build up global quality of life for patients with breast cancer who underwent modified radical mastectomy.
