单中心达芬奇机器人胃癌根治术140例临床分析

目的探讨达芬奇机器人在胃癌根治术中的应用效果,总结手术经验。方法回顾性分析2014年1月至2016年11月期间在解放军总医院普外科接受机器人胃癌手术的患者140例临床资料。结果 140例机器人胃癌根治术均成功,其中根治性远端胃大部切除术98例,根治性近端胃大部切除术10例,根治性全胃切除术32例,无1例中转开腹。手术平均时间247 min,平均术中出血量118 ml,平均清扫淋巴结32枚。术后平均进流质饮食时间3.8 d,术后平均住院时间7.5 d,手术后并发症11例(7.9%)。结论达芬奇机器人胃癌根治术创伤小,安全可行,近期临床疗效好。

Experience of 140 Cases of da Vinci Robotic-assisted Surgery for gastric cancer

Objective To investigate the clinical outcome of gastrectomy by using da Vinci robotic surgical system and to summarize surgical experience.Methods Clinical data of 140 patients with gastric cancer which received robotic gastrectomy from Jan 2014 to Nov 2016 were reviewed.Results All of 140 cases underwent robotic gastrectomy successfully,including 98 cases of radical distal gastrectomy,10 cases of radical proximal gastrectomy,32 cases of radical total gastrectomy,without conversion.Mean operation time was 247 min,mean blood loss was 118 ml,mean number of harvested lymph nodes were 32.Mean time for taking liquid food was 3.8 d,mean hospitalization was 7.5 d,postoperative morbidity was 7.9％.Conclusion Radical surgery for gastric cancer with da Vinci robotic system is feasible and safe,with minimal trauma and good short-term clinical outcome.
