血管介入与全身化疗治疗中晚期肺癌疗效分析

目的通过支气管动脉热灌注化疗与全身静脉化疗对比,分析二者在中晚期肺癌治疗中的临床近期疗效及不良反应。方法共入组198例中、晚期非小细胞肺癌患者,采用seldinger技术,对其中102例进行选择或超选择性灌注丝裂霉素、顺铂加环磷酰胺,对其余96例进行吉西他滨加顺铂全身化疗。结果经2个周期治疗后,可以看出支气管动脉灌注化疗较全身化疗的的疗效佳（P〈0.05）,而经4个周期治疗后2组的疗效无明显差别（P〉0.05）,支气管动脉灌注化疗较全身静脉化疗的毒副反应小（P〈0.05）。结论支气管动脉灌注治疗中晚期肺癌较全身静脉化疗近期疗效佳,不良反应小,远期疗效有待进一步研究。

Vascular Interventional and Systemic Chemotherapy for Advanced Lung Cancer

Objective Bronchial artery perfusion chemotherapy and systemic chemotherapy was compared to analyze the clinical curative effect and adverse reactions in the advanced lung cancer.Methods 198 cases of advanced non-small cell lung cancer patients were treated with seldinger technology.Among them,102 cases were treated with selective or super-selective perfusion of mitomycin,cisplatin plus cyclophosphamide.96 cases were treated with gemcitabine plus cisplatin.Results After two cycles of treatment,we found better efficacy of bronchial arterial infusion chemotherapy for advanced lung cancer patients(P<0.05),whereas there were no significant differences between the efficacy of the two groups after four cycles (P>0.05),but compared with systemic intravenous chemotherapy,the toxicity of bronchial arterial infusion chemotherapy was smaller(P<0.05).Conclusion Compared with systemic intravenous chemotherapy,bronchial artery infusion therapy had a better short-term effect and smaller adverse reaction in advanced lung cancer patient,but the long-term effect remains to be studied further.
