甘精胰岛素在2型糖尿病患者围手术期的临床应用

目的:探讨甘精胰岛素在2型糖尿病患者围手术期治疗中的疗效。方法:62例围手术期2型糖尿病患者分为胰岛素泵治疗组(CSII组)23例,常规胰岛素治疗组(MSII组)18例,甘精胰岛素治疗组(G组)21例。观察各组患者治疗前后血糖变化、血糖调整时间、5d达标率、胰岛素用量、低血糖发生率及术后并发症发生率。结果:各组治疗后血糖均明显低于治疗前,G组治疗后血糖低于MSII组,与CSII组无明显差异;G组低血糖发生率低于MSII组,而与CSII比较无显著性差异。结论:甘精胰岛素在糖尿病患者围手术期降糖效果显著,能迅速、平稳控制血糖,有望作为部分糖尿病患者围手术期的胰岛素泵替代治疗。

Application of glargine in perioperative patients with type 2 diabetes

Objective:To determine therapeutic effect of glargine in perioperative patients with type 2 diabetes.Methods:Sixty-two perioperative patients with type 2 diabetes were treated with insulin pump(CSII group,n=23),multiple subcutaneous insulin injection(MSII group,n=18),and glargine injection(G group,n=21).The blood glucose,insulin dose and hypoglycemia incidence rate were observed.Results:The levels of blood glucose in all groups were reduced after the treatments,but the level in G group was lower than that in MSII group.The incidence rate of hypoglycemia in G group was lower than that in MSII group,but similar to that in CSII group.Conclusion:Glargine can rapidly and stably control the blood glucose level in perioperative patients with type 2 diabetes,so it may substitute for insulin pump in some patients.
