5种清洁手术预防性应用抗菌药物调查分析

目的:了解清洁手术抗菌药物的应用情况,为抗菌药物的合理应用和管理提供参考。方法:回顾性调查分析某三甲医院5种清洁手术共153例患者抗菌药物的使用情况。结果:153例患者围术期抗菌药物的使用率为100%,两种抗菌药联用占46.4%;预防用药共4类,按使用频次的多少依次为头孢菌素类、青霉素类、硝基咪唑类、氟喹诺酮类;术前0.5～2 h内给药者10例,术前>2 h给药8例,术后给药135例;手术后停药时间:术后1～2 d 17例,3～7 d 130例,7～15 d 6例。结论:在清洁手术预防性应用抗菌药物中存在指征掌握不严,选用品种不当,盲目联用,给药时机欠佳,用药时间过长等不合理现象,围术期抗菌药物的使用有待进一步规范。

Investigation and Analysis on Antibiotic Use for 5 Kinds of Aseptic Operations

OBJECTIVE：To investigate the antibiotic use for aseptic operations and to provide references for the rational use and management of antibacterials.METHODS：We retrospectively studied the antibiotic use for 5 kinds of aseptic operations in a total of 153 cases.RESULTS：Of the total 153 patients,the perioperative antibiotic use rate was 100% and the combination of two kinds of antimicrobial drugs accounted for 46.4%.There were a total of 4 categories of preventive medications,which ranked in descending order according to use frequency were cephalosporins,penicillins,nitroimidazole and quinolones.10 cases were administered with drugs within 0.5-2 h before operation,8 cases 2 h before surgery,and 135 after operation.Drug withdrawal in 17 cases occurred 1-2 d,130 cases 3-7 d and 6 cases 7-15 d after operations.CONCLUSION：Irrational phenomena were noted in prophylactic antibiotic use for aseptic operations such as the non-strict mastering of the medication indication,improper selection of drug variety,blind drug combination,inappropriate administration occasion,prolonged medication duration etc.The perioperative antibiotic use remains to be further standardized.
