颅脑手术患者预防性应用抗癫痫药分析

目的:分析预防用抗癫痫药在颅脑手术患者中的使用情况。方法:收集颅脑手术患者的病例资料,设计调查表对患者疾病类型、手术情况、药物用法用量及患者用药依从性等情况开展随访调查。结果:完成调研的125例病例中,患者平均年龄51.2岁,平均手术时间为3.8 h。使用药物主要为丙戊酸钠和卡马西平,一般术前即开始预防使用药物,疗程多为3～6个月,而患者用药依从性较差,严格按医嘱规律全程用药的患者不足三分之一。结论:对于颅脑术后不建议长期预防性使用抗癫痫药,应强调个体化给药,定期监测,保障患者用药安全。

Analysis of Prophylactic Use of Antiepileptic Drugs in Patients Undergoing Craniocerebral Operations

OBJECTIVE：To analyze the prophylactic use of antiepileptic drugs in patients undergoing craniocerebral operations.METHODS：We collected the case records of the patients undergoing craniocerebral operations and designed questionnaire for follow-up investigation on the patients regarding disease type,operation conditions,administration and dosage,medication compliance etc.RESULTS：Of the 125 cases investigated,the mean age of them was 51.2 years and the mean operation time was 3.8 h.The drugs used were mainly sodium valproate and carbamazepines.The preventive drug use often started before operation with treatment course of over 3-6 months mostly.Patients had poor medication compliance,with less than 1/3 of the patients strictly following the medical order in the whole course of administration.CONCLUSION：Long-term prophylactic use of antiepileptic drugs is not recommended for patients undergoing craniocerebral operations.Individualized administration and regular monitoring should be emphasized to ensure medication safety.
