双水平气道正压治疗重叠综合征并呼吸衰竭

目的 探讨夜间睡眠时应用经口/鼻面罩双水平气道正压通气(BiPAP)对重叠综合征并呼吸衰竭患者的治疗价值.方法 选择住院的重叠综合征并呼吸衰竭患者42例,随机分为2组,治疗组在常规治疗基础上(包括氧疗 药物等)夜间给予BiPAP,对照组仅常规治疗.观察并对比2组疗效、动脉血气变化及并发症的发生率、住院时间、住院病死率等.结果 2组治疗3 d后动脉血气(PaO2、PaCO2)有显著差异(P＜0.05),治疗7 d 2组的总有效率、显效率、气管插管率、住院时间差异均有显著性(P＜0.05).结论 BiPAP治疗重叠综合征并呼吸衰竭比单纯常规治疗显效快、并发症少,可减少气管插管率及住院病死率、缩短住院时间,疗效肯定,值得临床推广.

The therapeutic effects of bilevel positive airway pressure ventilation on overlap syndrome complicated with respiratory failure

Objective To investigate the therapeutic effect of bilevel positive airway pressure ventilation(BiPAP) on overlap syndrome complicated with respiratory failure.Methods 42 patients with overlap syndrome complicated with respiratory failure were randomly divided into 2 groups.The patients in observation group were treated by BiPAP when they were sleeping,on the basis of routine treatment(oxygen therapy+drugs).The patients in control group were treated only by routine treatment.The therapeutic effects,changes of oxygen content in arterial blood,the hospitalization time,mortality in hospital and incidence of complications were observed and compared for the two groups.Results After 3-day's treatment,there was a significant difference in the change of oxygen content of arterial blood(PaO_2,PaCO_2) between two groups(P<0.05);after 7-day's treatment,there were significant differences in intotal effective,effective rate,trachea intubation rate and hospitalization time between two groups(P<0.05).Conclusion BiPAP therapy is effective in treating overlalp syndrome complicated with respiratory failure,which can decrease complications,trachea intubation rate,mortality and hospitalization time.
