高血压病人腹部外科手术围手术期处理体会

目的探讨合并高血压的腹部外科手术病人围手术期的治疗措施。方法对2000年-2006年我院收治的86例进行腹部外科手术的高血压病人的临床资料作回顾性分析。结果1级高血压28例，2级高血压40例，3级高血压8例，单纯收缩期高血压10例；合并心肌缺血30例，左心室肥厚20例，术后发生心肌梗塞1例，无死亡病例。结论高血压病人行腹部外科手术治疗时应联合内科做好术前评估和充分的准备，尽量将血压控制在正常高值水平，术中尽量保持血压平稳，术后严密监测控制血压，以减少心肌梗塞、心衰等并发症的发生。

Abdomen surgery complicated with hypertension in perioperative period's treatment

Objective To explore the abdomen surgery comlicated with hypertension in perioperative period＇ s treatment. Methods From 2000 to 2006, 86 patients complicated with hypertension who were performed with abdomen surgery in our hospital were analyzed retrospectively. Results There were 28, 40 and 8 cases were in degree 1,2 and 3 respectively. 10 cases were isolated systolic hypertension. There were 30 and 20 cases were complicated with myocardial iscbemia and left ventricular hypertrophy respectively. 1 case were complicated with myocardial infarction in the postoperative period. There was no patient died of operation. Conclusion Before the patients complicated with hypertension undergo abdomen surgery treatment, preoperative evaluation with other departments and enough preparation should be performed to control the blood pressure to the normal level. Maintaining the steadiness of blood pressure in the perioperative period and supervising the blood pressure in the postoperative period are vital to lessen the opportunity of myocardial irtfarction, heart failure and other complications.
