膀胱造影诊断18例创伤性膀胱破裂

目的探讨创伤性膀胱破裂的膀胱造影方法。方法回顾性分析了18例患者的临床资料,对膀胱造影的诊断方法进行研究。经导尿管注入15%~20%泛影葡胺250 mL,拍摄膀胱正位、左后斜位及右后斜位片。如无造影剂外溢,继续注入造影剂150 mL后,再拍摄膀胱正位、左后斜位及右后斜位片。如无造影剂外溢,则延时10 min后拍摄膀胱正位、左后斜位及右后斜位片。排空造影剂后拍摄膀胱正位片。结果全组18例均手术,其中腹膜内破裂5例,腹膜外破裂8例,混合型破裂5例。膀胱造影阳性16例,其中延时后,发现造影剂外溢1例。2例术前未能明确诊断,经手术证实为腹膜外膀胱破裂1例,腹膜内型膀胱破裂1例。结论逆行膀胱造影是诊断膀胱破裂简单而可靠的检查方法 。
