摘    要:目的:探讨新生儿窒息患儿血清心肌肌钙蛋白Ⅰ(cTn—Ⅰ)水平及临床意义。方法:采用全自动化学免疫系统检测40例新生儿窒息患儿(轻度窒息、重度窒息)血清cTn—Ⅰ和肌酸磷酸激酶心型同功酶(CK—MB),并与10例健康新生儿进行对照比较。结果:各组窒息新生儿血清cTn—Ⅰ浓度明显升高,轻度窒息组、重度窒息组两两之间及与对照组相比均差异显著(P<0、05)。窒息组血清中cTn—Ⅰ浓度与CK—MB浓度呈显著正相关(r=0.669,P<0.05)。窒息组中有心血管并发症与无心血管并发症患儿血清cTn—Ⅰ浓度比较有显著差异(P<0.05),且重度窒息组中两者差异更显著(P<0.01)。轻度窒息组和重度窒息组中有心血管并发症组患儿血清cTn—Ⅰ浓度比较差异显著(P<0.05);而无心血管并发症组急儿血清cTn—Ⅰ浓度相比无显著差异(P>0.05)。结论:cTn—Ⅰ可作为新生儿窒息所致急性心肌损伤的辅助诊断指标。

Abstract:Objective:To investigate the clinical significance of serum cardiac troponin I(cTn-I)level after asphyxia neonatorum.Methods:The serum cTn-I and the CK-MB of 40 patients with asphyxia neonatorum(mild asphyxia\,severe asphyxia)were detected by full-automatic chemo-immune system,and were compared with 10 cases of health new-born.Results:The concentration of serum cTn-I in various asphyxia neonatorum groups increased obviously,the mild asphyxia group compared with the severe asphyxia group,and these two groups compared with the control group,there were significant difference among them(P<0.05).The concentration of serum cTn-I and the CK-MB in asphyxia group had notable positive correlation( r =0\^669,P<0.05).The concentration of serum cTn-I in asphyxia group with cardiovascular complication compared with the asphyxia group without cardiovascular complication,there were significant difference between them(P<0.05),and there were notable difference between them in severe asphyxia group(P<0.01).The concentration of serum cTn-I in severe asphyxia group with cardiovascular complication were higher than that in mild asphyxia group with cardiovascular complication,there were significant difference between them(P<0.05),but there were no difference between two groups without cardiovascular complication(P>0.05).Conclusions:The cTn-I might act as an auxiliary index to diagnose the acute myocardial damage suffering from asphyxia neonatorum.
