后路椎间盘镜治疗腰椎间盘突出症疗效分析

目的 评价腰椎间盘镜治疗腰椎间盘突出症的疗效。方法回顾分析129例采用腰椎间盘镜髓核摘除术治疗的腰椎间盘突出症患者，男71例，女58例；年龄22～71岁，平均37岁。病变部位：L3/4。7例，L4/5 66例，L5/S1，43例，L4/5和L5/S113例。评估术后半年和3年的疗效及满意度。结果术后半年和3年JOA评分由术前平均（6．20±1．30）分，分别提高到（12．40±1．22）分和（13．75±1．65）分（P〈0．01）；VAS评分由术前平均（7．30±1．25）分分别下降至（1．78±0．64）分和（2．07±0．76）分（P〈0．01）。术后平均3年优107例，良12例，可10例，差0例。患者自我评价满意度分别为92％。平均恢复工作时间2个月。结论后路椎间盘镜具有创伤小、出血少、术后恢复快的优点是治疗老年腰椎间盘突出症有效且安全的手术方法。

Analysis of Curative Effect on Discectomy with Diskoscope

Objective To estimate clinical curative effects of lumbar intervertebral disc herniation on discectomy with diskoscope. Methods A hundred and twenty-nine patients with lumbar intervertebral disc herniation were retrospectively analyzed. There were 71 males and 58 females. The age was from 22 to 7 lyears old with an average of 37 years. The affected region of patients were L3/4（7 cases）, L4/5 （66 cases）, LS/S 1 （13cases）, L4/Sand L5/S1 1 （13 cases）. The curative effects were followed up and estimated by recording satisfaction degrees at half a year and 3 years（average） after operation. Results A halfa year and 3 years（average）after operation, the scores by JOA standard improved from 6.20 ± 1.30 to 12.40 ± 1.22, 13.75± 1.65（P 〈 0.01） respectively; the scores byVAS standard were reduced from 7.30 ±1.25 to 1.78±0.64, 2. 07± 0.76 （P 〈0.01 ） respectively; the excellent and good rates were 107 cases and 12 cases respectively; the satisfaction degree of the patients was 92%. The average time before working was 2 months. Conclusion Discectomy with diskoscope has a good clinical effect, little blood loss and rapid recovery, which is an effective and safe method for treating the senile lumbar disc herniation.
