咽旁间隙肿瘤61例临床分析

目的：探讨临床较罕见的咽旁间隙肿瘤的手术治疗。方法：对我科2000年6月2005年12月收治的61例咽旁间隙肿瘤患者临床资料进行回顾性研究。结果：咽旁间隙肿瘤中，以神经源性肿瘤居多（27例），其次为混合瘤（16例），余为先天性颈侧囊肿（4例），颈静脉球体瘤（1例），血管瘤（5例），恶性肿瘤（8例）。除颈静脉球体瘤患者放弃手术治疗外，其余均予手术治疗。其中2例术后复发予再次手术，1例淋巴瘤术后予放疗，2例恶性肿瘤因复发和远处转移而死亡。结论：手术治疗咽旁间隙肿瘤目前取得良好疗效，尤以颈侧切开术广泛应用于咽旁间隙肿瘤治疗，经口径路仅用于较小的肿瘤切除，上颌骨切开术只用于范围较大侵及颅底的手术。

Analysis of 61 Cases of Parapharyngeal Space Tumour

Objective:To study the surgical operation on scarce parapharyngeal space tumor scarce. Methods:61 patients who were diagnosed with parapharyngeal space tumor during December 1999 to December 2005 were aralyzed restrospectively. Re-suite: Among all patients with parapharygeal space tumor,27 patients with neuroriginal tumor, followed by 16 patients with mixed tumor, and 4 patiens with congenital branchial cleft cyst, one with glotnus jugular tumor, 5 patients with hemangioma, other 8 patients with malignant carcinoma. Except one patient with glomus jugular tumor who gave up therapy,others were treated by surgical operation. 2 patients recured after operation and accepted operation again,one patient with lymphadenoma accepted radiopraxis, 2 patients with malignant carcinoma died of metastasis and recurrent. Conclusion: Surgical operation on parapharyngeal space tumor can get very good curative effect, especially, lateral neck dissection is widely used. Transoral approach operation is adapted to small tumor. Maxilla dissection is only fitted in with abroad skull base encroachement.
