去氧孕烯炔雌醇片与米非司酮对围绝经期功能失调性子宫出血的疗效比较

目的比较米非司酮与去氧孕烯炔雌醇片(妈富隆)治疗围绝经期功能失调性子宫出血的临床疗效。方法将围绝经期功能失调性子宫出血患者120例随机分为米非司酮组(A组)和去氧孕烯炔雌醇片组(B组),治疗随访3个月,比较两组调经效果、治疗前后子宫内膜厚度、药物副作用。结果治疗3个月后,米非司酮组在治疗期间有效止血、无复发,去氧孕烯炔雌醇片组停药后有撤退性出血,复发率为21.67%;米非司酮组血清激素水平卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、孕酮(P)较治疗前下降,去氧孕烯炔雌醇片组E2及P较治疗前下降;两组子宫内膜厚度均较治疗前变薄。结论米非司酮治疗围绝经期功能失调性子宫出血较去氧孕烯炔雌醇片疗效确切,值得推广。

Mifepristone Combined with Desogestrel and Ethinylestradiol Tablets in Treating 60 Cases of Perimenopausal Dysfunctional Uterine Bleeding

Objective To observe the clinical effect of mifepristone combined with Desogestrel and Ethinylestradiol Tablets（marvelon） in the treatment of perimenopausal dysfunctional uterine bleeding. Methods 120 cases of perimenopausal dysfunctional uterine bleeding were randomly divided into two the mifepristone group and Desogestrel and Ethinylestradiol Tablets group. The treatment and the 3 months follow-up were performed. The regulating menstruation effects, endometrial thickness and side effects of the drugs after treatment were compared between the two groups. Results After 3 months treatment, the mifepristone group had effective hemostasis without re- currence during treatment period, while the Desogestrel and Ethinylestradiol Tablets group had withdrawal hemorrhage and the recurrence rate of 21.67% after stopping drug; the hormone levels of FSH, LH, E2, P in the mifepristone group were decreased than before treat-ment and the levels of E2 and P in the Desogestrel and Ethinylestradiol Tablets group were decreased compared with before treatment; the endometrial thickness in the two groups were thinker than that before treatment, the difference had statistical siguificance（P〈0.05）. Conclusion Mifepristone has more definite effect than Desogestrel and Ethinylestradiol Tablets in the treatment of perimenopausal dys- functional uterine bleeding and is worth promoting in clinic.
