2012年1月至6月医院静脉用药调配中心不合理医嘱分析

目的规范临床用药,保障患者用药安全。方法收集2012年1月至6月的医院静脉用药医嘱,根据药物使用方法、注意事项等对不合理医嘱进行分析。结果共审核出不合理医嘱3 372份,其中配伍禁忌766份,药物超量978份,浓度过大895份,溶剂选择不当675份,未成组医嘱58份。结论药师应充分发挥审核并纠正不合理用药,促进临床安全、合理用药。

Analysis of Irrational Medical Orders in PIVAS of Our Hospital from January to June 2012 and Solution

Objective To standardize the clinical drug use for ensuring patient＇s medication safety and effectiveness.Methods The medical orders of intravenous medication in the Pharmacy Intravenous Admixture Service（ PIVAS） from January to June 2012 were collected and analyzed on the irrational medical orders from the aspects of the usage,matters needing attention,etc.Results Three thousand three hundred and seventy-two irrational medical oders were examined and verified,in which,766 medical orders were incompatibility,978 medical orders were excessive dosage,895 medical orders were excessive concentration,675 medical orders were improper selection of solvent and 58 non-grouping medical orders.Conclusion Pharmacists should fully play the role of examination and verification and correct irrational medication orders for promoting rational drug use in clinic.
