纤维支气管镜局部治疗联合全身使用抗生素对肺脓肿的疗效

目的　探讨纤维支气管镜对肺脓肿的局部治疗作用。方法　对照组 2 6例 ,予常规内科治疗 ;纤支镜治疗组 3 2例 ,在对照组治疗基础上行纤维支气管镜 (纤支镜 )局部灌洗并局部给药。结果　纤支镜治疗组 2周、4周、6周累积治愈率分别为 2 1.9%、5 6.3 %、90 .6% ,对照组 2周、4周、6周累积治愈率分别为 3 .9%、15 .4%、42 .3 %。纤支镜组的 2周、4周有效愈率有高于对照组显著差异 (P <0 .0 5 ) ,而 6周累积治愈率两组无显著差异 (P >0 .0 1)。结论　纤支镜局部治疗联合内科常规治疗较单纯内科常规治疗显效快、治愈率高 ,是一种安全、简单、有效的方法

The Effect of Local Medication with Branchofiberoscope Combined with Injecting Antibiotics Intravenously in Patients with Lung Abscess

Objective To discuss the curative effect of local treatment with branchofiberoscope on lung abscess. Methods 58 patients were divided into local medication group (32 patients) and control group (26 patients) randomly. Routine medical treatment was used in the control group. In addition to routine medical treatment, Bronchoalveolar lavage and local medication were used in the local medication group. Results The cure rates in the local medication group in 2, 4, 6 weeks were 21.9%, 56.3% and 90.6% respectively, in the control group, there were 3.9%, 15.4%, 42.3% respectively. The difference of the cure rate in 2 ,4 weeks was significant ( P <0.05) between two groups; and in 6 weeks, it was no significant ( P >0.01) between them. Conclusions The medical treatment with the addition of local medication is safe, simple and effective. The cure rate and curative effect of local medication group were better than simple routine medical treatment.
