电视腹腔镜和后腹腔镜手术在同时治疗多器官病变中的应用

目的：探讨腹腔和后腹腔镜在治疗多器官病变中的应用价值．方法：腹腔镜和后腹腔镜治疗15(男10，女5)例患有多器官病变的患，年龄31～66(平均41)岁，其中左侧多房性肾囊肿、结石性胆囊炎1例，左肾盂肿瘤、结石性胆囊炎1例，双侧肾上腺皮质增生2例，多囊肾5例，双侧多发性肾囊肿6例．结果：除1例肾囊肿因肾周粘连转开放手术外，其余14例均获成功．术后皮下气肿2例．随访3～15mo，生活质量良好，1例肾囊肿复发．肾盂癌患术后12mo无肿瘤复发．结论：腹腔镜下同时行多脏器手术较开放手术更有显优势，具有损伤小，出血少，术后疼痛轻，并发症少，恢复快和良好的切口外观等优点．

Application of laparoscopy and posterior laparoscopy in treatment simultaneously for multi-organ diseases

AIM: To investigate the value of laparoscopy and posterior laparoscopy in the treatment of multi organ diseases. METHODS: Laparoscopic and posterior laparoscopic procedures were performed in 10 male and 5 female patients (31 to 66 years old, mean age 41) with multi organ diseases. Of the 15 cases, 1 case suffered from multilocular cysts of left kidney and lithic cholecystitis, 1 case from tumor of left renal pelvis and lithic cholecystitis, 2 cases from bilateral adrenal cortex hyperplasia, 5 cases from polycystic kidney and 6 from bilateral muitiple cysts of kidney. RESULTS: 14 cases were successful except for one case of polycystic kidney due to extensive adhesion. Subcutaneous emphysema occurred in 2 cases. After 3 to 15 month follow up showed that cyst of kidney relapsed in 1 case and 12 month follow up showed that carcinoma of renal pelvis did not recur. CONCLUSION: Laparoscopy and posterior laparoscopy, with minimal trauma and postoperative discomfort and short hospital stay, are the first choice approach in the treatment of multi organ diseases.
