认知行为干预对维持性血液透析患者生活质量的影响研究

目的探讨认知行为干预对维持性血液透析患者生活质量的影响。方法维持性血液透析患者180例随机分为两组：干预组和对照组,对照组行常规血液透析护理,干预组在对照组基础上加入认知行为干预,分别在干预前后发放焦虑自评量表（SAS）、抑郁自评量表（SDS）以及简明健康调查问卷,评定两组患者人体学测量体重指数（BMI）,免疫比浊法测定前白蛋白（PA）。结果干预组SAS和SDS评分明显低于对照组（P〈0.05）;干预后除RP外,其它SF-36各项因子评分明显高于对照组（P〈0.05）;干预组患者BMI和PA评分均明显高于对照组（P〈0.05）。结论认知行为干预可以明显缓解维持性血液透析患者的焦虑抑郁等负性情绪,提高患者生活质量,改善患者的营养水平。

Study the Effect of Cognitive-behavioral Therapy for Maintenance Hemodialysis Patients' Quality of Life

Objective Research on the effect of cognitive-behavioral therapy for maintenance hemodialysis patients′ quality of life.Methods Maintenance hemodialysis patients 180 cases were randomly divided into two groups:control group and experimental group.The control group was received routine nursing care of hemodialysis.The experimental group was received routine care plus cognitive-behavioral intervention.Before and after intervention,the patients were investigated by questionnaires,Self-rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS) and Short Form 36 Health Survey Questionnaire(SF-36),and the patients in two groups were detected the Body Mass Index(BMI),PA.Results After mental intervention,the results of SAS and SDS in experimental group were significantly lower than those in control group(P<0.05);Except the score of RP under SF-36,the other SF-36 factors score were significantly higher than those in the control group(P<0.05);BMI and PA in experimental group were significantly higher than those in control group(P<0.05).Conclusion Cognitive-behavioral intervention can significantly ease negative feelings of anxiety and depression with the maintenance hemodialysis patients.It can also improve both the quality of life and the nutritional status of patients.
