骨髓活检在骨髓穿刺干抽患者中的诊断意义

目的探讨骨髓活检在骨髓穿刺干抽患者中的诊断意义。方法采用骨髓活检塑料包埋技术,对5 148例骨髓常规穿刺呈干抽现象的54例患者的骨髓活检结果进行了回顾分析。结果 54例骨髓穿刺干抽患者均得到明确诊断。骨髓穿刺干抽发生率为1%,发生率较多的有原发性骨髓纤维化（100%）、骨髓转移癌（33%）、慢性粒细胞白血病（12%）、再生障碍性贫血（12%）等;发生骨髓穿刺干抽的54例患者性别差异不大,年龄大者发生率高,且均有不同程度的网状纤维的增多;骨髓穿刺干抽的发生与骨髓增生异常活跃和极度减低也有一定的关系。结论骨髓活检能为骨髓穿刺干抽患者的诊断和治疗提供明确依据,也是常规骨髓细胞形态学检查的有力补充。

Diagnostic value of the bone marrow biopsy for patients with dry tap

Objective To explore the diagnostic value of the bone marrow biopsy for patients with dry tap.Methods There were 54 of 5 148 bone marrow aspirations having dry tap.For these cases,bone marrow biopsies were examined with plastic embedding technology,and the results were analyzed retrospectively.Results The 54 cases of dry tap aspirations were all diagnosed definitely.The incidence of dry tap aspiration was 1%.The causes of dry tap included primary myelofibrosis（100%）,metastatic carcinoma（33%）,chronic myeloid leukemia（12%） and aplastic anemia（12%）.There were no significant differences among all the dry tap cases in sex.The elder cases presented high incidence and showed different degrees of reticulin fibre proliferation.There was a correlation between the occurrence of dry tap and the degree of bone marrow proliferation.Conclusions The bone marrow biopsy provides definite information for the diagnosis and therapy of patients with dry tap,and it serves as a powerful complement for bone marrow cellular morphology.
