广泛性焦虑患者焦虑程度与心率变异性相关研究

目的探讨广泛性焦虑(GAD)患者焦虑程度与心率变异性(HRV)的关系。方法选取6O例GAD患者,根据焦虑程度分为3组,进行汉密顿焦虑量表(HAMA)总分及躯体性、精神性焦虑因子分的测定,同时采用24 h动态心电图仪测定HRV指标。将躯体性、精神性焦虑因子分与HRV指标进行相关分析。结果 3组患者躯体性焦虑因子分、精神性焦虑因子分比较,差异均有统计学意义(P<0.05);重度组患者躯体性焦虑因子分与精神性焦虑因子分比较,差异有统计学意义(P<0.05)。躯体性焦虑因子分与24 h每5 min正常R-R间期的标准差的平均数(SDANN)、低频功率(LF)、低频与高频功率之比(LF/HF)均呈正相关(P<0.05);精神性焦虑因子分与正常相邻R-R间期差值的均方根(RMSSD)、相邻R-R间期≥50 ms的心搏数占总心搏数的百分比(PNN50)呈负相关(P<0.05)。结论 HRV指标与GAD患者焦虑严重程度存在一定相关性。

Study of Relationship between Anxiety Level and Heart Rate Variability in Generalized Anxiety Disorder

Objective To investigate the correlation between anxiety level and heart rate variability(HRV) in generalized anxiety disorder(GAD).Methods According to anxiety level,sixty GAD patients selected were divided into three groups and were detected about the overall score of HAMA and somatic anxiety factor score and psychic anxiety factor score,at the same time they were underwent 24 hour ambulatory electrocadiographic recordings to obtain the data of HRV.Finally,a correlation analysis was made between somatic anxiety factor,psychic anxiety factor and HRV index.Results The differences in somatic anxiety factor and psychic anxiety factor between the three groups were all significant(P<0.05);The difference between somatic factor and psychic anxiety factor in severe group was also significant(P<0.05).Somatic anxiety factor was positively correlated with SDANN,LF and LF/HF(P<0.05);Psychic anxiety factor was negatively correlated with RMSSD and PNN50(P<0.05).Conclusion Some correlation was existed between HRV index and severity of anxiety in GAD patients.
