腹腔温热灌注化疗晚期胃癌66例临床观察

目的　通过对手术后 66例晚期胃癌 (Ⅳ期 )的前瞻性腹腔温热化疗药物灌注治疗 ,观察治疗效果和副反应。方法　 66例胃癌中探察手术 12例 ,姑息切除 5 4例 ,平均术后 8天开始腹腔周期性温热化疗。结果　 66例随访 3、6、12、2 4个月 ,远处转移和死亡例数分别为 0、1;9、9;14、2 1;2 8、44 ;平均中位生存 16 5个月 ,最长随访 48个月 5例存活 ,仍随访中。结论　腹腔温热灌注化疗适用于晚期胃癌 ,术后辅助温热化疗药物灌注治疗 ,能有效的抑制肿瘤的生长发展 ,有效杀灭腹腔内的游离癌细胞和微小癌转移灶 ,可缓解病情、预防和治疗腹腔复发 ,提高术后生存率 ,改善生存质量 ,无严重副作用

Clinical Observation of 66 Cases with Advanced Gastric Cancer Using Intraperitoneal Hyperthermia Chemoperfusion

Objective To evaluate the clinical efficacy and side_effect of intraperitoneal hyperthermia chemoperfusion (IHCP) for patients with advanced gastric cancer Methods Of 66 patients, 12 underwent alone explorative operation and others underwent palliative resection All patients received IHCP periodicity beginning average 8th day after operation Results 3, 6, 12, 24_month followed_up showed the distant metastatic rates were 0/66, 9/66, 14/66, 28/66, respectively, the mortality rates were 1/66, 9/66, 21/66 and 44/66 respectively The median survival time was 16 5 months Conclusions IHCP can effectively depress tumor growth and development, effectively kill dissociated cancer cell and mini metastatic foci, increase survival rate and quality of life
