摘    要:目的:探讨乳腺癌保乳手术局部缺损即刻修复的适应证、方法、效果及可行性。方法:对37例乳腺癌患者行保留乳房手术后应用背阔肌肌皮瓣(Latissimus dorsi myocutaneous flap,LDM)和邻近皮瓣(adjacentflap)即刻乳房缺损修复。结果:手术均取得成功。LDM修复22例,邻近皮瓣修复15例,术后随访6月至3年以上乳房形态自然,效果满意,外观评价可接受率100%。结论:Ⅱ期及部分ⅢA期患者行保乳手术并即刻乳房修复是安全可靠,其外形自然,顺应性佳且能够保留原有乳房皮肤的感觉,从而获得最佳美容效果。

Abstract:Objective:To investigate the indication,method,effect and feasibility of the immediately partial repair with breast conserving surgery.Methods:Latissimus dorsi myocutaneous flap(LDM) and adjacent skin flap were used for the breast repair and construction in 23 cases of breast cancer who received breast conserving surgery and axillary lymph node dissection.Results:LDM was applied in 18 cases,and adjacent skin flap in five cases.All breast construction operations were successful with natural appearance and satisfied results.The acceptable rate was 100% for the post-operative ocular estimate.Conclusion:With its excellent cosmetic results,the immediately partial repair with breast conserving surgery in selected patients was safe and reliable.
