体表胃电动态频谱及皮肤电反应在脾胃病证患者的再观察

摘    要:应用“微电脑胃肠电检测分析系统”观测152例脾胃虚实患者和63名对照组健康人的体表胃电和皮肤电,胃电信号以10min为一个时段作动态频谱分析。结果显示:1.单纯脾虚患者空腹胃电活动量(安静期228±14,活跃期629±40单位)低于正常(安静期333±18,活跃期1119±140单位),餐后10min内胃电达到最大活动量的百分例数(48.1%)及活动量(500±70单位)显著低于健康人(67.3%,731±60单位),餐后胃电活动指数曲线较正常低平并后移,提示空腹胃运动减弱,进餐反应延缓并减弱,可解释纳差、食后腹胀等症状的部分病理机制。此外,本组患者刺激时的皮肤电反应亦显著低于正常,说明脾虚患者有效负荷情况交感和副交感神经的兴奋能力都不足,处于失代偿期。2.脾虚肝郁与肝郁脾虚兼胃气上逆患者空腹胃电处于活跃期的百分序列数(20.0%,17.5%),两证患者尤其兼胃气上逆者空腹胃电活动指数均明显高于正常,提示空腹胃运动亢进及有关调控因素异常,并可说明患者恶心呕吐等症状的部分病理机制。3.上述两证患者餐后10min内胃电达到最大活动量的百分例数(54.2%,55.6%)类似单纯脾虚者而低于健康人,活动指数曲线后移,但餐后10min内胃电达到的最大活动量则明显高于单纯脾虚者,甚至高于健康人,可作鉴别。此外,患者受刺激时的皮肤电反应亦明显高于单纯脾虚者而与健康人相似,提示有效负荷情况交感和副交感神经的兴奋能力处于代偿期,与单纯脾虚患者不同。

Abstract:Electrogastrogram (EGG) and electroskinic activity (ESA) of 152 patients with Pi-wei syndrome and 63 healthy adults as control were examined, and the running spectrum of EGG were analysed by Fast Fourier Transform. The results showed that: 1. compared with healthy control, fasting EGG activit ofy patients with spleen-qi deficiency is weaker, the activity index curve of postprandial EGG lower and delayed, and amplitude of ESA in stimulated condition lower; 2. fasting EGG activity of patients with liver-qi stagnation and spleen-qi deficiency is higher than that of the control.
