全甲状腺切除术在治疗甲癌中的作用

１９９４年８月～１９９８年７月，行全甲状腺切除术治疗甲癌６９例。其中分化型甲癌５９例（ＰＴＣ５４例、ＦＴＣ５例），髓样癌７例、未分化癌３例。３７例已浸润外周组织，临床治愈５５例（７９．７％）。由于分化型甲癌具有高分化性及多中心性的特点，故全甲状腺切术具有彻底切除病灶，避免残癌，为早期诊断及用＾１３１Ｉ治疗分化型甲癌转换灶及复发作好充分准备等优点，讨论了手术指征及禁忌症。作者认为，只要由有丰富经验的

The Efficacy of Total Thyroidectomy in the Treatment of Thyroid Carcinoma

Form Aug 1994 to Jul 1998,69 total thyroidectomise (TT) were performed for thyroid carcinoma (TC) in this hospital.Fifty-nine with differentiated thyroid carcinoma (DTC) (papillary 54,follicular 5),7with medullary thyroid carcinoma and 3 undifferentiated thyroid carcinoma.In the 37 cases,peripheral tissues were infiltrated with cancer cells had been found during operation.Fifty-five (79.7%)were cured clinically.AS DTC was charalterized with distinctive features of well-differentiation and multicentricity,so TT could remove cancer tissues thoroughly and aviod local residual carcinoma,and was helpful in early detection of local recurrence and distant metastases and early postoperative administration of 131 I.This paper discussed the indications and contra-indications of TT.The authors believe that low risk and well-effectiveness are possible if the operation is performed by well-experienced specialized surgeon.
