色度学原理在牙齿美白修复及口腔临床比色中的应用与评估

目的：研究牙齿美白剂的增白效果,建立并运用数码照相计算机色度分析系统来评价分析牙齿美白效果;并初步建立牙体软硬组织色彩学评价指标以探讨二者在色度学方面的协调性。方法：将40例受试者随机分配到3个实验组,分别经过3种不同临床实验处理共计640颗着色前牙,运用Vita比色板和数码照相计算机色度分析软件,分析各组实验前后比色板色值和色差值△E。同时进行切牙区牙龈软组织相关色度值的采集与分析,对牙体软硬组织色彩的协调性进行初步分析与探讨。结果：冷光美白组（A组）牙面Vita比色板色值实验前后比较相差非常显著（P〈0.01）,而单纯漂白组（B组）和凡士林空白对照组（C组）实验前后比色板色值比较相差并不显著（P〉0.05）;A组分别与B组和C组比较其色差值△E相差非常显著（P〈0.01）,B组和C组比较其色差值△E相差并不显著（P〉0.05）。下颌附着龈与游离龈在明度学指标上无显著差异,却存在红绿黄蓝等饱和度指标的统计学差异;上颌游离龈较附着龈明度为高,且牙龈色彩偏红偏黄,饱和值增大。结论：运用Beyond冷光牙齿美白技术的确具有美白牙齿的疗效,近期效果肯定。牙龈软组织在口腔牙体硬组织临床比色及缺损修复的选配色方面有重要参考价值,尤其上颌前牙在美白修复中更应关注软硬组织的色彩协调与统一。

The clinical application and assessment in teeth whitening and oral rehabilitation research by colourometric principles

Objective To study the whitening effect of tooth whitening agent,establish and apply the digital photography computer analysis system to evaluate the effect of teeth whitening method,And to build the preliminary tooth color evaluation indicators systems of soft and hard tissue in order to explore their compatibility of chroma.Methods 40 subjects were randomly assigned to three equal experimental groups,with a total of 640 stained anterior teeth was processed by three different clinical trials.Then we analyzed the values of CIE color system and the color difference △E of the experimental group before and after the experiment while using vita shade guide and color analysis software computer with digital camera and at the same time incisor gum chroma values associated soft tissue were collected and analyzed so as to do the preliminary analysis and discussion of color coordination about soft and hard tissue of dentistry.Results In which,Beyond whitening group the color values of its Vita color board of tooth surface fluctuate significantly before and after the experiment（P0.01）,while the color values of simple bleaching and Vaseline control group before and after the experiment are almost remain the same（P0.05）;Beyond whitening group and a simple bleaching or Vaseline control group,their color difference △E varied significantly（P0.01） while color difference △E of a simple bleaching group and Vaseline control group have no significant difference（P0.05）.Mandibular attached gingival and free gingival indexes in the brightness has no significant difference,but there is Statistic difference of saturation indicator in color of red,green,yellow and blue.Lightness value of maxillary free gingival is larger compared to attachment,and its reddish yellow gum color with larger saturation value is rendered.Conclusion This clinical trial showed that through computer analysis system to deal with chroma analysis and the application of Beyond cold light tooth whitening technology does have the effect of teeth whitening.Besides,Gingival soft tissue has important reference value in the oral clinical color comparison and selection,especially in the maxillary anterior teeth whitening prosthesis,more concern should be paid to the color coordination and unification of soft and hard tissue.
