经桡动脉途径冠状动脉介入治疗临床分析

目的探讨经桡动脉途径进行冠状动脉介入治疗（TRA—PCI）的可行性、安全性和疗效。方法对52例冠心病患者行TRA—PCI，总结其疗效以及出现的并发症。结果52例冠心病患者中，其中单支病变15例，双支病变21例，三支病变16例；处理前降支（LAD）28处，回旋支（LCX）15处，RCA20处，其中完全闭塞2例，血管狭窄程度在70％～99％，共放置支架65枚，单纯PTCA2例，桡动脉穿刺成功率85％，TRA—PCI成功率92％；术后6h拔除桡动脉鞘管加压包扎。2例出现前臂血肿，经保守治疗后痊愈。无死亡及急诊冠脉搭桥。结论TRA—PCI治疗具有血管并发症少，安全可行，方便患者，减少痛苦，应积极提倡经桡动脉途径进行冠状动脉介入治疗。

Clinical Analysis of Transradial Approach for Coronary Intervention

Objective To evaluate the feasibility, safety, and success rate of transradial approach for coronary intervention. Methods The clinical and angiographic results after coronary intervention by transradial approach in .52 consecutive patients were retrospectively analyzed. Results Among the .52 selected patients, there were 1.5 patients with single vessel lesion, 21 patients with couple vessel lesion, and 16 patients with three vessel lesions. 6.5 stents were implanted for .52 cases. PTCA was only performed for 2 cases. The success rates of radial artery puncture and TRA--PCI procedure were 8.5 % and 92% respectively. The complication of anterior arm hematoma occurred in 2 cases, and no death and emergency CABG was reported. Conclusions Transradial approach for coronary intervention is safe and feasible with a high success rate and lower complications.
