乳腺癌动态增强MRI特征:非肿块样强化与肿块样强化的比较

摘    要:目的 对非肿块样强化与肿块样强化乳腺癌的动态增强磁共振成像(DCE-MRI)特征进行比较.方法 收集269例经穿刺活检证实为乳腺癌,且在化疗前或手术前接受乳腺MRI检查的病例.根据强化类型分为肿块样强化病变组及非肿块样强化病变组,比较两组动态增强参数差异性.结果 269例乳腺癌患者中,非肿块样强化28例(10.41%),肿块样强化241例(89.59%).50.00%非肿块样强化乳腺癌增强后晚期达到强化峰值,而81.74%肿块样强化乳腺癌早期即达强化峰值(P<0.001).39.29%非肿块样强化乳腺癌呈流入型曲线,42.86%呈流出型曲线;75.93%肿块样强化乳腺癌呈流出型时间一信号强度曲线,仅9.13%呈流入型曲线.两种强化类型病变的流出率、信号增强比(SER)亦存在统计学差异.结论 与肿块样强化乳腺癌比较,非肿块样强化乳腺癌倾向于增强后缓慢强化、呈现流入型时间信号强度曲线.
