67例股骨下端骨折的手术治疗

目的总结股骨下端骨折的受伤机理及其治疗方法。方法对67例患者根据骨折的不同类型、损伤程度采取不同治疗方法。结果67例均手术治疗,67例随访6个月～4年(平均2年5个月)。骨折愈合65例,骨不连2例,按Merchan等的功能评定标准,优39例,良15例,中10例,差3例。优良率80.6%。骨不连2例于术后1年植骨内固定。创伤性关节炎8例,2例膝关节伸直位强直。结论对股骨下端骨折的治疗关键是骨折的准确复位,恢复关节面平整和膝关节功能最大限度的保留。术后早期不负重功能锻炼,减少创伤性关节炎的发生,膝关节腔透明质酸钠(SH)的应用,有利于膝关节功能的恢复。

67cases of inferior segment of femoral bone fractrue treated by Operation

Objective To summarize the injury mechanism and treatment of inferior segment of femoral bone fracture. MethodTo adopt different treatments according to different type and degree of fracture. Result 67 cases were treated by operationand visited by 6 months to 4 years .(average 2 years and 5 months).65 cases fully recovered from fracture ,2 cases weredisunion.According to the function evaluation standard of merchan ect ,39are first-rate,15first-rate,15are second-rate,10 are general,2 are bad.The fineness rate is 80.6%,The 2 cases which were 8cases of traumatic arthritis,2 cases of kneejoint rigidity. Conclusion The key treatment in inferior segment of femoral bone fracture is to exactly reset,recover articularsurface and remain knee joint function to a maximum,and not to bear a heavy burden in early functional exercise aftersurgery to reduce the risk of traumatic arthritis. Using knee joint Sodium Hyaluronate (SH)is good for recorery of kneejoint function.
