碘-125放射性粒子在头颈部恶性肿瘤中的应用

目的 探讨碘-125（125Ⅰ）粒子植入内放射治疗技术在头颈部晚期恶性肿瘤治疗中的临床效果。方法采用术中植入、B超定位经皮穿刺植入、鼻窦内窥镜引导下放置等方法对16例头颈部恶性肿瘤患者在术后残腔、肿瘤内或转移淋巴结内放置碘-125粒子。结果 近期观察无局部感染、无放射性损伤、无血管破裂出血、无粒子移位或脱落等状况发生，半年完全缓解率62．5％，1年完全缓解率37．5％。结论 碘-125粒子植入内放射治疗技术可成为头颈部恶性肿瘤患者的综合治疗方法之一，此技术尤其适合于术后复发、放疗后复发、一期手术难以根治等晚期恶性肿瘤患者，亦适合于需行术后放疗的早期恶性肿瘤患者。

Application of 1251 seeds brachytherapy in head and neck malignant tumor

Objective To evaluate the brachtherapy effectiveness of 125I seeds implanted in the head and neck malignant tumor. Methods 125I seeds were implanted in wound area, inside tumors or inside lymph nodes of 16 patients with head and neck malignant tumor through operation, B ultrasound guided puncture or endoscope of nasal sinuses guided implantation. Results The brachtherapy of 125I seeds in malignant tumor can improve curative effect. No local infection, acute radioactive lesion,vessel broken or seeds migration happen. The complete relieve rate of half year was 62.5% and one year 37.5% . Conclusion The 125I brachytherapy can used be one of the comprehensive treatment in head and neck malignant tumors, especially the recur tumors of postoperation, post - radiotherapy or the terminal cancer which cann' t be radically treated by first operation. It' s also can used in early stage carcinoma who need postoperative radiotherapy.
