高频CDFI超声对乳腺肿块诊断及定位的临床价值

摘    要:目的:探讨高频彩色多普勒显像(CDFI)超声对乳腺肿块诊断及定位的临床应用价值。方法:对186例乳腺肿块进行高频CDFI超声检查,其中154例为乳腺结构不良回声图,余32例为乳腺肿块回声图。本文主要对32例经高频CDFI检查诊断及定位,并经手术、病理证实的乳腺肿块进行回顾性分析。结果:高频CDFI诊断32例肿块中,囊性7例,实质性22例,多发性囊性肿块3例。良性肿块23例,恶性肿块9例。与手术病理对照肿块检出率100%。对肿块的囊性、实质性、混合性的检出率也是100%。良恶性肿块除1例与手术病理不符外,总符合率为96.9%(31/32)。高频CDFI超声对乳腺血管彩色血流的检出率为72%(24/32)。良恶性肿块的动脉血流阻力指数有显著性差异,P<0.01。高频CDFI检查定位准确与触诊比较有明显差异,P<0.05。手术证实,全部肿块均在超声定位象限内。结论:高频CDFI超声检查对乳腺肿块的性质,良恶性鉴别诊断以及定位,对临床手术均有指导意义。此检查方法应做为乳腺肿块诊断的首选方法之一。

Abstract:Objective:To evaluate the clinical value of high frequency CDFI for the diagnosis and localization of breast masses.Methods:One hundred eighty six cases with breast masses were examined by high frequency CDFI.Of the 186 cases,154 cases with mammary dysplasia and 32 cases with breast masses were found in high frequency CDFI respectively.The diagnosis and localization of 32 cases confirmed surgically and pathologically were reviewed.Results:Of the 32 cases,7 cystic,22 parenchymatous,and 3 multiple cystic masses were involved in this observation.There were 23 cases of benign bereast masses and 9 cases of malignancy.The result of high frequency ultrasonography was compared to surgery and pathology.The detected rate of breast masses was 100%,and the detected rate of cystic,parenchymatous or mixed masses was 100% espectively.The total coincidence rate of benign and malignant masses was 96 9%(31/32),and only in one cases,the diagnosis was not correspondent to that of surgery and pathology.In high frequency CDFI,blood flow signal detected rate was 72%(24/32).There was a significant difference between the RI of malignant breast masses from that of benign ones( P <0 01).It was more accurate to locate breast masses with high frequency CDFI in comparison with palpation( P <0 05)The locations of the breast masses shown in the high frequency sonograms were confirmed in surgery.Conclusions:High frequency CDFI which provides significant guides to the characteristics and the localization of the breast masses should be the first useful and the chief alternative means used to the differential diagnosis and selecting clinical operation for breast messes.
