支气管肺泡灌洗术治疗炎性肺实变疗效观察

目的探讨应用支气管肺泡灌洗术（BAL）治疗炎性肺实变的疗效。方法选择X线胸片或胸部CT诊断为肺实变且经痰培养确诊为炎性肺实变共99例。随机分为对照组46例,观察组53例,二组均按常规静脉使用抗生素,口服茶碱、祛痰药等治疗,观察组同时行BAL治疗,每周2～3次。结果治疗后根据临床症状的变化、X线胸片或CT的改变以及血常规、痰培养改善等情况判断疗效。观察组显效17例,有效30例,总有效率达88.68%。对照组显效4例,有效16例,总有效率为43.48%。二组总有效率比较,P〈0.05,差异有统计学意义。BAL治疗炎性肺实变效果显著。结论在常规治疗基础上给予BAL治疗炎性肺实变,可有效提高炎性肺实变治愈率。

CURATIVE EFFECT OF BRONCHOALVEOLAR LAVAGE ON INFLAMMATORY CONSOLIDATION OF LUNG

Objective To discuss the curative effect of bronchoalveolar lavage on inflammatory consolidation of lung. Methods 99 cases were chosen with the diagnosis of lung consolidation by sternum or chest CT and sputum culture, who were randomly divided into the control group with 46 cases and the treatment group with 53 cases. The two groups were given conventional antibiotics by vein, aminophylline by oral and expectorants by oral as well. In addition, BAL treatment was given to the treatment group 2-3 times per week. Results The curative effects were evaluated by clinical symptoms, sternum or CT changes, routine blood test and sputum culture as well. In the treatment group, 17 cases had marked effect, 30 cases had efficient, total effective rate was 88.68%. While in the control group, 4 cases had marked effect, 16 cases had efficient, total effective rate was only 43.48%. There were significant difference between the effective rates of the two groups （P〈0.05）. Conclusion Based on the conventional treatment, BAL therapy can treat the inflammatory lung consolidation, and raise the effective rate.
