导乐仪与椎管内麻醉分娩镇痛疗效对比观察

目的比较导乐仪和椎管内麻醉两种镇痛方法对分娩的作用与影响。方法将2011年2月至2012年2月于来潍坊市妇幼保健院分娩的产妇320例,随机分为导乐仪组与椎管组各160例。导乐仪组应用导乐仪镇痛,椎管组行椎管内麻醉镇痛。观察两组的镇痛效果、产程时间、分娩方式、新生儿评分、产后出血量并作比较。结果椎管组麻醉镇痛效果优于导乐仪组,导乐仪组产程时间短于椎管组,导乐仪组经阴平产率高于椎管组,差异有统计学意义(P<0.05);两组新生儿评分及产后2 h出血量比较,差异无统计学意义(P>0.05)。结论导乐仪与椎管内麻醉镇痛效果都比较理想,对产妇和新生儿无不良影响,选择何种方法镇痛,须尊重产妇意愿,综合分析具体应用。

Comparative Study on Doula and Intraspinal Anesthesia against Birthing Pains

Objective T o compare the two analgesia methods′ effects of doula and intraspinal anesthesia on childbirth.Methods 320 cases randomly selected from Weifang Maternal and Child Health Hospital during Feb.2011 to Feb.2012 were randomly divided into two group,the doula group and the spinal group,160 case each.Doula group were given doula delivery analgesia,the spinal group were given intraspinal anesthesia analgesia.The maternal analgesia,duration of labor,mode of delivery,postpartum hemorrhage,neonatal score of the two groups were observed.Results Intraspinal anesthesia analgesia effect was better than that of doula group,doula group′s labor time was shorter than that of the spinal group,virginal delivery rate of doula group was higher than spinal group,the difference was statistically significant(P<0.05).The score of newborn and two hours postpartum hemorrhage volume of the two groups were not statistically significantly different(P>0.05).Conclusion Doula and intraspinal anesthesia analgesia effects are both ideal,without adverse effects on the maternal and neonatal.The maternal intention should be respected in the choice of analgesia with a comprehensive analysis of the specific application.
