舌根癌手术治疗2例报告

目的 :探讨手术治疗舌根癌的方法。方法 :分别为 2例舌根癌患者实施咽侧入路及下颌骨正中裂开外旋入路切除肿瘤 ,应用胸骨舌骨肌肌筋膜瓣和胸大肌肌皮瓣修复缺损。结果 :2例患者切口均一期愈合 ,经锻炼逐步恢复了吞咽及语言功能 ;术后 10月和 18月复发。结论 :术前充分检查 ,准确评估肿瘤范围是选择合适的术式 ,彻底切除肿瘤的前提 ;咽侧入路及下颌骨正中裂开外旋入路是切除舌根癌最常用的有效手术径路 ;胸骨舌骨肌肌筋膜瓣和胸大肌肌皮瓣修复是两种有效的修复方法 ,前者对术后吞咽、语言功能的影响较小 ,后者可修复局部较大的组织缺损

Surgical treatment of carcinoma of the base of tongue

Objective: To explore the surgical ways for carcinoma of the base of the tongue . Methods: 2 cases with cancer of the tongue base was performed surgery by two accesses of pharyngotomy and submaxillary split.And the defect was repaired with sternohyoid muscle and fascia flap and pectoralis major myocutaneous flap respectively. Results:The incision was healed without infection,the function of swallow and speech recovered after training.Recurrence happened after 10 and 18 months respectively.Conclusion:Perfect preoperative examinations,the exact assessment for the tumor range is important to choose the appropriate surgery means and to resect tumor completely. Pharyngotomy and submaxillary split approach are the common and efficient surgery ways. Sternohyoid muscle and fascia flap and pectoralis major myocutaneous flap are the two effective reconstructive methods, the former affect the swallow and speech function little,the latter can repair large tissue defect.
