糖尿病患者根管治疗中疼痛有关因素的临床研究

目的 探讨糖尿病患者在根管治疗过程中发生根管治疗中疼痛(endodontic interappointment pain, EIP)的有关因素.方法 以临床需行根管治疗的79例糖尿病患者和82例非糖尿病患者各86颗患牙为研究对象,按患者血糖控制好坏分组,分析其与正常组EIP发生率、疼痛程度及疼痛在术后不同时间发生的相关情况.结果 血糖控制差组EIP发生率、术后不同时间出现疼痛以及疼痛程度严重的比率明显高于控制好组和正常组(P＜0.05);而控制好组与正常组之间差异没有统计学意义(P＞0.05),但3组内不同疼痛分级的发生率均随其程度加重而减小.结论 糖尿病患者根管预备术后7 d内都有可能发生疼痛,良好地控制血糖有利于减少EIP发生率和减轻疼痛程度.

Correlation of diabetes mellitus and endodontic interappointment pain

Objective To investigate the correlation of diabetes mellitus (DM) and endodontic interappointment pain (EIP) in root canal therapy (RCT). Methods Seventy-nine DM patients and 82 patients without DM were recruited, 86 teeth required RCT in each group. DM patients were subdivided into two groups by the level of fasting plasma glucose (FPG): poorly controlled group if FPG>9.0 mmol/L, and well controlled group if FPG<7.8 mmol/L. The correlation of EIP incidence rate, algesic degree and algesic time after RCT was statistically analyzed. Results The EIP incidence rate and the incidence at given time in poorly controlled group were higher than well contolled and non-DM groups (P<0.05). The algesic severity of poorly controlled group was more than well controlled and non-DM groups (P<0.05). No statistic differences were between well controlled and non-DM groups (P>0.05). However, in each group, the incidence rate of different algesic severity decreased with the increasing algesic severity. Conclusion EIP will occur in DM patients within a week after RCT. Well controlling of the blood glucose can reduce the incidence rate of EIP.
