基层医院在诊治消化性溃疡合并出血方面的差距

目的 分析基层医院与三级甲等医院在消化性溃疡合并出血诊治方面的差距.方法 选择2000年1月-2010年6月因消化性溃疡合并出血入住北京市普仁医院(普仁医院)治疗的61例患者为病例组,按成组匹配原则选取北京大学第三医院(北医三院)住院的122例患者为对照组,回顾性分析两组胃镜检查,幽门螺杆菌、肿瘤标记物检测,治疗及预后情况.结果 两组胃镜检出率、幽门螺杆菌检出率、肿瘤标记物检查率、质子泵抑制剂使用率比较,差异均有统计学意义(P＜0.01).两组住院期间再出血率比较,差异无统计学意义(P＞0.05);而便隐血阴转时间分布比较,差异有统计学意义(P＜0.05).结论 基层医院在急诊胃镜检查率、药物治疗以及幽门螺杆菌的检测及根除幽门螺杆菌治疗方面与三级甲等医院相比均有差距,三级甲等医院患者住院期间便隐血转阴时间较短.
