腹腔镜子宫肌瘤剥除术84例分析

目的 评价腹腔镜下子宫肌瘤剥除术的临床价值。方法 2009年6月至2011年9月成都市妇女儿童中心医院行腹腔镜下子宫肌瘤剥除术84例和开腹子宫肌瘤剥除术68例,比较两组手术时间、术中出血量、术后肛门排气时间及住院时间及肌瘤剥除率。结果 腹腔镜下子宫肌瘤剥除术术中出血量明显少于经腹子宫肌瘤剥除术,其手术时间和术后肛门排气时间均明显短于开腹手术,差异有统计学意义(P<0.05);腹腔镜下剥除肌瘤数目占术前B超检测数目的94%,而开腹手术为100%,两组差异有统计学意义(P<0.05)。结论 腹腔镜下子宫肌瘤剥除术具有创伤小、手术时间短、术中出血少、术后恢复快等优点,但不能完全替代开腹手术。

Clinical analysis of Laparoscopic Myomectomy in 84 cases

Objective To analyze the clinical value of laparoscopic myomectomy.Methods The data from operation time,blood loss volume,anal exhaust time of 84 cases with laparoscopic myomectomy and 68 cases with abdominal myomectomy in Chengdu Central Hospital of Women and Children in Sichuan Province between June 2009 and September 2011.Results The blood loss volume in laparoscopic myometomy was significantly lower than those in abdominal myomectomy,the operation time and exhaust time after operation in laparoscopic myomectomy were significantly shorter than that in abdominal myomectomy that the difference was significant(P<0.05).The total number of laparoscopic stripping fibroids was 94% of the total number detected by preoperative ultrasound,while open surgery was 100% and the difference was no statistically significant(P>0.05).Conclusion Laparoscopic myomectomy is with less trauma,less bleeding,rapid postoperative recovery etc,superiority in a certain extent laparotomy,but it cannot completely replace the open surgery.
